High Fidelity Wraparound

Program Overview

High Fidelity Wraparound (HFW) is an evidence-based, family-driven approach designed to support youth experiencing behavioral health challenges. Rather than relying solely on formal social service systems, HFW focuses on increasing the use of natural supports already present in the youth’s world. 

The young person and their family guide a highly collaborative team that includes extended family members, schools, churches, coaches, and other trusted individuals. By bringing these supports together, the team helps meet the youth’s needs in a way that feels personal, sustainable, and rooted in the community.

Currently serving Venango County, High Fidelity Wraparound has shown strong outcomes. Youth involved in the program have experienced reductions or elimination of truancy, stronger connections within their communities, and improved relationships and positive behaviors at home, in school, and in the community.

Population Served

Youth ages 8 to 21 with:

  • Complex behavioral health needs or a behavioral health disorder
  • Diagnosis of serious emotional disturbance or substance abuse disorder
  • Youth who are at risk of being placed out of the home
  • Youth who are in, or nearing discharge from, an out-of-home placement treatment facility or hospitalization
  • Youth who have complex emotional and behavioral needs
  • High-end users of Behavioral Health Services

The Team

The High Fidelity Wraparound team consists of a supervisor/coach, facilitator, family support partner, and youth support partner.

Supervisor/Coach

The coach is an accomplished facilitator with good interpersonal skills and experience supporting development in others. They have a broad base of experience and possess a diverse view of what families need to live better lives. The coach collaborates closely with supervisors in other child-serving agencies in the community.

Facilitator

The facilitator ensures the values and steps of the process are delivered with the highest possible fidelity to national best practices, while still allowing for local individualization of the process. The facilitator develops a partnership with each family that helps address and work through challenges to make positive changes in their lives

Family Support Partner (FSP)

An FSP provides intensive levels of direct support for families in need. Their role is distinct from that of the facilitator and youth support partner, but they work closely with both to obtain outcomes at the family level. FSPs may be a program graduate or a family member of a person with complex emotional or behavioral health needs. FSPS are there to support the family as a team member when needed, model strength-based approaches, and share experiences as teachable moments.

Youth Support Partner (YSP)

The YSP position provides intensive levels of direct support for youth who want it. Like the FSP, they may be a program graduate. They may also be a young adult with experience managing their own behavioral health issues.

The YSP assures that the Hi-Fi team members hear and gain better insight into the youth voice and culture.

The Motto of the family support partner and the youth support partner is: “Do for, do with, cheer on,” meaning that at times we need to help the family with direct support, do with the family, and walk beside them as they learn to do for themselves, and cheer them on when they do the tasks on their own.

Program Impact

High Fidelity Wraparound received a referral for a young person in February 2026 due to school attendance concerns, including 66.5 unexcused absences and explosive outbursts toward teachers. During discovery, the youth and grandmother shared that school avoidance began after the death of the youth’s mother from an overdose, and miscommunications compounded the outbursts.

Recognizing the importance of education, the youth and grandmother worked with the Wraparound team and school to build a support plan: grief counseling through the SAP program, permission to visit the principal’s or guidance office when overwhelmed, tutoring, grief workbooks to complete together, and stronger natural supports within the school. The functional assessment also identified a need to strengthen coping skills and reduce reactive anger. These efforts demonstrate how personalized support can lead to meaningful progress and hope for families.

With these supports in place, the youth caught up on missed work and graduated on time. The team continues to support the family as the youth transitions into adulthood, working toward goals such as obtaining a driver’s license and building a career path.


Referrals to High Fidelity Wraparound come from a wide range of sources. These include:

– Child, youth, and family services
– Juvenile justice system
– School and educational professionals
– Drug and alcohol counselors
– Other behavioral and mental health providers

Referrals are also welcomed from family members and other caring adults, including church leaders, teachers, coaches, and more.
