Intensive Aftercare and Community Monitoring

Program Overview

The Intensive Aftercare and Community Monitoring Programs are designed to support youth who are at risk of further involvement with the juvenile justice system. The goal is to reduce the need for out-of-home placement by helping young people remain connected to their families and access the supports they need. Each child’s plan is individualized to their specific needs, strengths, and risk factors. 

Serving Venango County, MHY’s programs focus on keeping youth in their homes and communities whenever it is safe to do so. These programs provide close supervision, guidance, and follow-up services in the home and throughout the community to help young people stay on track and avoid additional offenses. 

Services are delivered in line with the standards set by the Juvenile Court Judges Commission for intensive monitoring and aftercare probation. This means youth are held accountable, while also being given the structure and support they need to make better choices and build healthier routines.

Highly trained specialists meet with youth in a variety of settings, including their homes, schools, and throughout the community. They maintain frequent contact, provide supervision, and help each child get involved in positive activities. This may include community service, volunteer work, or other structured, pro-social opportunities that encourage responsibility, accountability, and positive peer connections.

Program Impact

Brian struggled with a severe addiction to multiple drugs and experienced two overdoses. After significant effort, I was able to help him enter a residential treatment program for his substance use disorder. Upon completing treatment and being released, Brian graduated from high school with good grades, earned his journeyman card through the Ironworkers Union, and secured a full-time job with benefits.

Brian remained sober and was consistently employed, demonstrating remarkable progress and stability. Of all the youth I worked with, he was one of the individuals I feared might not survive his addiction. By the time of his discharge, Brian was also living independently and managing his life successfully.

Youth eligible for these programs are Venango County juveniles who are typically referred by a judge or through the juvenile probation office.

Through consistent support, real-world engagement, and strong community connections, the Intensive Aftercare and Community Monitoring Programs work to help children avoid deeper involvement in the system and move toward a more positive future.
